  1. Go to https://doxy.me/ through your google or safari browser on your IPhone, IPad, laptop or computer with camera.

  2. Pick "Get started for free".

  3. Pick "I'm a patient".

  4. Enter your provider's doxy.me link from below after "https://doxy.me/".  

  5. Click on "Check In".

  6. Enter your name.

  7. Click on "check in" again. 

  8. Enable camera/microphone if needed. 

  9. You will be entered into your provider's waiting room.

  10. Wait on your provider to come online for your appointment.

  • WALK-INS WELCOMED for established patients for SICK and EMERGENCY visits ONLY!

  • Call your Pharmacy first for medication refills!

  • Form completion could take up to 10 business days. 

  • Controlled substance refills could take up to 72-hours to be processed.

1)IF YOUR BALANCE IS OVER 30 DAYS OLD interest may be charged at 8% per annum.  

​

2) We now have a NO-SHOW POLICY .

 If you no-show for two appointments, you will be notified that you will need to pay a non-refundable amount of $25 before you can schedule another appointment. 

​

3) We have a LATE POLICY. If you are over 15 minutes late for your appointment, you will need to reschedule your appointment for another time or day.
